You can’t see this in Face Select Mode, but I suspect that you have Double Vertices; maybe you Inset once and then you tried again to Inset with Outset enabled, but that immediately hits the outer rings that were created with the first Inset.
That’s a lot of words, clumsily trying to explain this…
Just select all and Merge by Distance.
