That looks like normal behavior. I’m guessing you expected the smooth brush to smooth out the entire surface while maintaining volume of the sculpture automatically. However the smooth brush doesn’t maintain volume and therefore has a secondary function as a flatten brush. So when you’re trying to apply the smooth brush to a large area, you often get pinches and bumpiness.
You need to smooth the bumps out one by one.
